The last step we need to complete before we can set up our date is verification. Screening is unavoidable and mandatory for all new friends.

Please fill out my reservation form accurately and in its entirety. You may also submit your screening information to my encrypted email at “cheriefields@protonmail.com”. Please choose a screening method listed below.

I. References

Submit 2 references from providers that you have seen in the last 6-8 months. Please include their name, website, & email.

II. Employment Verification

Occupation, company name, active LinkedIn profile, and work email/ number.

Further screening beyond these methods may be required in order to schedule a date. Negotiation of my screening process will not be tolerated. Your privacy is very important to me and all personal information will be terminated after the verification has been completed. I can also sign an NDA upon request.
